Description In the globalization era like today, information technology progressing rapidly. This is evident from the emergence of computer technology and internet development. To handle student satisfaction questionnaire accurately and quickly, information system is needed by STMIK Global. This system will process the result of questionnaire from students so that the error in the processing of input answer will be minimized. The method used for this research is System Development Life Cycle (SDLC) with object oriented analyze and development approach start from analyze the running system through unified Modeling Language (UML). The programming language used was PHP and database used was MySQL. Questionnaire system, called Satisfaction Application System is the result of this research. Besides that, this research also generated server local host web-based application which is could increase operational performance especially for academic department’s staff.
